Louisiana Medicaid Waiver Services
A Hand To Hold, LLC offers the following Waiver services supportive care:
+ Now Opportunities Waiver (NOW) offers people age 3 and older who otherwise would require the level of care given in a facility for the Developmentally Disables, to remain in their honk and communities. Services are based on the needs of the recipient and developed Using personalized individualized plan.
+ Children's Choice Waiver (C)
Offers supplemental support to children through age 18 who currently the at home with their families or with a foster family.
• supervised Independent Living services (SI) are provided in the residential setting allow the individual to the independently
and function within their own community. This program covers persons age 21 years
and older.
+ Community Choke Waiver:
The Community Choice Waiver Program provides services in the home and in the community to elders or adults with disabilities who qualify.

Personal Care Services In addition to Waiver services, A Hand To Hold, LLC, offers the following Medicaid
• Early & Periodic Screening Diagnostic Treatment
+ Personal Care Services (EPDST-PCS)
These services are offered to individuals who receive Medicaid and are ages 0 to 21 years. The services are proved in the comfort of the individual’s place of residence, and include assistance with, or the preparation of meals, and all household chores that are essential for the individual to live in a clean and safe environment while participating in the activities of daily living
+ Elderly and Disabled Adult Waiver: (LT-PCA)
Waiver Program provides Long-Term -Personal Care Attendant (supervision or assistance) with basic self-care activities and tasks such as dressing, bathing, preparing meals, grocery shopping, light housekeeping and laundry chores: accompany to medical appointments, provide are-alone personal assistance in the home or community to elderly or disabled adults who qualify. This program does not provide 24 hour per day
